Photo/Media Release Form

I hereby grant permission for my child to be included in the photographs, videos and other recording/publications, made in connection with St. Thom Church and/or the Archdiocese of Louisville, KY for a period of one calendar year. I understand that once the photo has been published it becomes part of th parish archives and may be used in the future.

Libaility Statement

I understand there is the possibility/ risk of injury/or loss which my child may sustain while participating in activites during scheduled Youth Ministry events.  I also understand that all precautions will be taken to ensure the safety and security of my child at all times during these activities. 

I hereby release and agree to hold harmless St. Thomas Parish and the Archdiocese of Louisville, KY and any of their volunteers, advisors, chaperones, or persons connected with the Parish Youth Ministry program from any liability, claims, damages for person injury, property loss/damage which may result during the course of the Youth Ministry year.

I further waive and release any and all claims against the owners, leasers or operators of any facilities or homes used for the meetings for any and all injuries or losses suffered by said child.

Medical Release Form

I hereby authorize the adult(s) and volunteer(s) representing St. Thomas Church and the Archdiocese of Louisville as my agents, to consent to any examinations, X-ray, anesthetic, medical, or surgical diagnosis or treatment and hospital care deemed advisable by a qualified physician or local hospital. A St. Thomas representative agrees to contact the undersigned as soon as possible if any emergency should arise. I will assume responsibility for fees incurred by such an emergency. In addition, I certify that the above information is correct and give permission for the release of medical records to the attending physician. I realize that I cannot hold the parish or the Archdiocese of Louisville responsible for such and emergency.

Social Media Release Form

St. Thomas Youth Ministry communicates with students about youth ministry activities and events via email, Instagram (@sttomym), Facebook (@sttomym), and the Remind App.

I hereby give permission for the pastor, youth minister, or other adult leaders to contact my child directly about St. Thomas Youth Group events and/or activities.
